Death of the Life Assured is to be notified to ING LIFE as soon as possible. The Death Intimation cum Claim Form may be used for this purpose; please attach all documents listed in Section C of this form.

ING LIFE - Claims department would on receipt of the claim intimation revert with further requirements, if any. The following is the list of primary documents to be submitted at intimation:

Original Policy Document
Original Death Certificate issued by Local Authority
Copy of Medical Cause of Death Certificate
Medical Records (Discharge / Death summary, Test results, Admission notes, Outpatient consultation notes, etc)
Age Proof & Identification document (with Photo) of Life Assured
Identification document (with Photo) of Claimant
Attested Copy of Post Mortem Report, if done
In case accident / unnatural death,
a) Jurisdictional Police Station Attested Copy of FIR
b) Jurisdictional Police Station Attested Copy of Panchnama /Inquest Report / Final Report
Proof of Bank account of Claimant (Self Attested copy of Bank Passbook / Account statement of Claimant)
Proof of Current Address of Claimant
