Accident Disability / Dismemberment Rider Claim |

Accident to the Life Assured which results in a Disability or Dismemberment is to
be notified to ING Life by Life Assured within 30 days from the date of accident.
The
ADDDB Claim Intimation Form may be used for this purpose. ING LIFE – Claims
department would on receipt of the claim intimation revert with further requirements.
